Job Summary

We are seeking a highly skilled Licensed Practical Nurse (LPN) to join our dynamic team at ReMed, a Collage Rehabilitation Partner. As a key member of our neurological rehabilitation team, you will play a vital role in providing exceptional care to adults with brain injury.

Key Responsibilities
  • Provide nursing and facilitation services for assigned clients under the direction of a registered nurse.
  • Contribute to and implement evidence-based treatment strategies that promote physical, psychosocial, and spiritual health.
  • Train clients or family members on how to promote health, wellness, and medical stability.
  • Collaborate with the interdisciplinary team to facilitate achievement of treatment plans and goals.
  • Contribute to the nursing assessment of clients and their families, as well as their educational and discharge needs.
  • Implement the interdisciplinary plan of care within the scope of LPN practice and provide nursing care and education directly or in collaboration with Brain Injury specialists.
Requirements
  • LA licensure as a LPN is required.
  • Experience in the rehabilitation of chronically or catastrophically ill or injured is recommended.
  • Demonstrated accountability and skills in analysis, problem solving, decision making, related to medical nursing skills and time management required.
  • The ability to communicate in writing and verbally with families, clients, staff, and physicians is a must.
  • A valid driver's license, insurance, and responsible driving behaviors must be maintained.
