Job Title: Licensed Practical NurseAbout the Role:

We are seeking a skilled Licensed Practical Nurse to join our team at ResCare Community Living. As a LPN, you will play a vital role in providing high-quality patient care to individuals with intellectual, developmental, or cognitive disabilities.

Key Responsibilities:
  • Plan and manage patient care according to individual needs, including interviewing patients and recording medical history and physical condition.
  • Obtain patient vital signs, including pulse, blood pressure, temperature, and respiration, and provide routine care for patients.
  • Provide 1:1 care for clients, following their plan of care while maintaining their dignity and independence.
  • Monitor clients' vital statistics and report abnormal findings to the case manager and physician.
  • Perform medication administration, feedings, oxygen, ostomy care as needed per client's plan of care and provide assessments of results.
  • Complete routines established by other care professionals, such as Physical Therapist, Occupational Therapist, and Speech Therapist.
  • Ensure infection control policies are maintained at all times and report possible communicable diseases or infections.
  • Accurately document nursing actions of all care given and communicate with family, case manager, and physicians.
  • Order supplies and medications as needed to ensure adequate amounts are on hand at all times.
  • Ensure all patient needs are being met in accordance with the instructions of the physician.
  • Monitor incidents involving individuals and ensure incident reports are completed timely and accurately.
Requirements:
  • Completed post-secondary coursework to facilitate licensure as a Licensed Practical Nurse or Licensed Vocational Nurse.
  • Knowledge of individual service plans, including understanding of client rights and health needs.
  • Proof of current license as a Licensed Practical Nurse or Licensed Vocational Nurse in good standing from the state(s) of practice.
  • CPR Certified.
  • Excellent interpersonal skills, including customer service, patience, empathy, and compassion.
  • In-depth understanding of patient care policies and regulations.
  • Familiarity with symptoms and signs of improvement for several medical conditions.
