Primary Care Physician Assistant

AAS Healthcare Staffing is seeking a dedicated Nurse Practitioner to join our team at our Outpatient clinic in Portsmouth, VA. This is a full-time opportunity with a competitive salary range of $110,000 - $165,000 based on experience and location, along with a quarterly bonus based on quality metrics.
About the Role:
- Provide exceptional primary care services to patients, including routine and acute issues, and home visits as needed.
- Administer annual wellness visits and health risk assessments, as well as coordinate care with other providers, specialists, testing facilities, and agencies.
- Lead population health initiatives in coordination with the Care Team, assisting with phone triage and outreach, and educating patients on their health conditions, care plans, and treatments.
Requirements:
- Candidates must hold national certification in one of the following specialties: Family Nurse Practitioner, Adult-Gerontology Primary Care Nurse Practitioner, Adult Nurse Practitioner, or Gerontological Nurse Practitioner.
- A current, non-probationary state Nurse Practitioner License is also required.
- Prior direct primary care experience as a Nurse Practitioner is necessary, along with an active DEA license.
