Your Role: As an Acute Care Licensed Practical Nurse specializing in Dialysis, you will provide both direct and indirect care to patients undergoing dialysis treatment for kidney ailments, under the guidance of the Dialysis Nurse Supervisor and Charge RN. Your responsibilities will include:
- Monitoring vital signs and administering medications.
- Documenting patient information accurately and providing educational resources to patients and their families.
- Operating dialysis equipment while adhering to strict infection control protocols to ensure patient safety.
The ideal candidate will possess excellent communication skills and work collaboratively within an interdisciplinary team, serving as a vital resource for fellow team members.
Key Responsibilities:
- Deliver personalized nursing care tailored to each patient's needs in the Dialysis setting.
- Accurately collect and record relevant health data, ensuring minimal exceptions as measured by supervisory reviews.
- Identify and report patient issues promptly, maintaining high standards of care.
- Implement and document delegated interventions with precision.
- Provide educational materials and conduct monthly reviews of lab information and home medications.
- Perform medical procedures in accordance with established nursing standards and hospital policies.
- Maintain meticulous documentation of patient interactions and care provided.
- Advocate for patient rights and ensure their autonomy and dignity are preserved.
Qualifications:
- Graduation from an accredited Practical Nursing program is required.
- Previous healthcare experience is preferred, but new graduates are encouraged to apply.
- Current NYS Licensure as a Practical Nurse or valid interim permit is mandatory.
- BLS certification must be obtained within six months of hire.

- Strong clinical skills and the ability to multitask effectively.
- Excellent communication and critical thinking abilities.
