Position Overview: The role is pivotal in managing the operations of a healthcare facility. The individual in this position will be tasked with various responsibilities and duties as required.

Key Responsibilities:

Ensure adherence to organizational and legal standards by evaluating and interpreting reports, implementing and overseeing safety measures, conducting operational and quality assurance assessments, formulating and executing action plans, and providing guidance on organizational programs and strategic objectives.
  1. Facilitates community outreach initiatives and motivates team members to engage positively with the community.
  2. Maintains confidentiality of sensitive information and records in accordance with organizational policies and applicable regulations.
  3. Demonstrates and guides team members on effective patient service techniques to ensure patient concerns are addressed within organizational standards.
  4. Collaborates with the Pharmacy Administrator and compliance team to ensure readiness for audits by various regulatory bodies.
  5. Ensures proper handling of hazardous waste and expired medications in compliance with relevant regulations.
  6. Accurately prepares medications for health center use and home delivery to meet patient health requirements promptly.
  7. Processes returned prescriptions efficiently to prevent potential insurance fraud.
  8. Provides comprehensive patient care by accurately dispensing medications, administering immunizations, counseling patients, and maintaining confidentiality of records.
  9. Adheres to pharmacy policies and formulary decisions to ensure uninterrupted patient care.
  10. Complies with state-approved programs that enhance pharmacist clinical roles under physician orders or collaborative agreements.
  11. Builds and nurtures collaborative relationships both internally and externally, fostering a respectful and inclusive work environment.
  12. Manages the ordering, receiving, and inventory of medications in line with accepted practices and corporate policies.
  13. Responsible for monitoring expiration dates and managing the return of near-expiration medications to suppliers.
  14. Performs additional tasks as assigned by the supervisor, which may be communicated verbally or in writing.
Additional Responsibilities:
  1. Ensures compliance with HIPAA regulations, maintaining confidentiality of all member information.
  2. Required to learn and utilize the Electronic Health Record and Practice Management System, including various electronic features as they are developed.
  3. All employees are expected to participate in the Patient Centered Health Home Model at Omni Family Health.
Qualifications, Education, and Experience:

EDUCATION:
  1. Completion of an accredited pharmacy program with certification or degree.
EXPERIENCE:
  1. Preferred minimum of 1 year of experience in a pharmacy-related role.
CERTIFICATION/LICENSE:
  1. Must possess a valid and active California pharmacy license.
  2. CPR certification is required.
SKILLS:
  • Interest in working within a community health center environment.
  • Completion of ACPE accredited immunization training when necessary.
  • Basic typing and computer skills, with the ability to enter and retrieve information.
  • Ability to communicate effectively with individuals and groups.
  • Capable of creating documents and reports as needed.
  • Ability to navigate through confined spaces and inspect equipment for proper functioning.
