Nurse Practitioner

We are seeking a highly skilled and compassionate Nurse Practitioner to join our team. As a primary care provider, you will be responsible for delivering evidence-based care to patients of all ages, from routine check-ups to managing chronic conditions. You will work collaboratively with our healthcare team to provide holistic care that meets the unique needs of each patient.

• Provide primary and family care to patients, including accurate assessment, diagnosis, treatment, and management of health problems
• Develop and implement individualized care plans that address the unique needs of each patient
• Collaborate with other healthcare professionals to ensure comprehensive care and coordination
• Perform routine examinations, administer immunizations, and order diagnostic tests as needed
• Provide education and counseling to patients on healthy lifestyle choices, disease prevention, and management

• Master's degree in Family Nurse Practitioner program with current National Board Certification and State of Employment license to practice in the Advanced Practice Nurse role
• At least two years of medically relevant experience or equivalent
• Basic Life Support (BLS) certification, including CPR and Automated External Defibrillator (AED) training
• Ability to speak and write in additional languages, including Spanish
