Qualifications

Minimum Requirements:

  • Active Pharmacy License in the relevant state
  • Not listed on the DEA Excluded Parties List
  • Immunization Certification from an accredited body (e.g., APhA)
  • Designated as the 'pharmacist in charge' on the state pharmacy license
  • Registered for PMP access where applicable
Preferred Qualifications:

Education:
A Bachelor of Science in Pharmacy or a Pharm. D. degree is preferred.
