Job Summary:

This position is located at the Minot VA Clinic in North Dakota. We are seeking a skilled Licensed Practical Nurse to join our Patient Aligned Care Team (PACT). The PACT LPN performs non-standard and specialized activities under the direction of a Registered Nurse, requiring a broad work experience and demonstrated skill sufficient to independently resolve a range of nursing care decision making with minimal periodic supervision by the Registered Nurse.

In this role, you will have the opportunity to work with a diverse group of patients, including those with physical and emotional needs. You will be responsible for performing various tasks, such as initiating, performing, and completing assigned duties in providing care to variable patient populations, observing for physical and/or emotional changes in patient's condition from prescribed medications/treatments, promptly and accurately documenting noted changes, and reporting any deviations from normal to RN or MD/DO.

Responsibilities:

  • Promptly and accurately document noted changes, and report any deviations from normal to RN or MD/DO.
  • Work collaboratively with the Registered Nurse and other healthcare professionals to develop and implement patient care plans.
  • Provide education to patients and their families on disease processes, medication management, and other health-related topics.
  • Maintain accurate and detailed records of patient interactions and treatments.
  • Participate in quality improvement initiatives to ensure the highest level of patient care.

Requirements:

  • Curently licensed as a Licensed Practical Nurse (LPN) in the State of North Dakota.
  • At least 1 year of experience as an LPN in a hospital, clinic, or other healthcare setting.
  • Strong communication and interpersonal skills.
  • Ability to work effectively in a fast-paced environment.
  • Basic Life Support (BLS) certification required.
