About the Position:
We are seeking dedicated Home Support Professionals to join our team and provide exceptional care to our clients.
Key Responsibilities:
- Providing direct care and support to clients in their homes, assisting with daily living activities such as bathing, dressing, grooming, and meal preparation.
- Monitoring clients' medications as prescribed by healthcare professionals, observing for any side effects or adverse reactions, and reporting concerns to appropriate healthcare providers.
- Implementing and following individualized care plans tailored to each client's specific needs, regularly reviewing and updating care plans in collaboration with healthcare professionals and family members.
- Monitoring clients' progress and behavior, keeping detailed records of their daily activities, health status, and any significant changes, maintaining accurate and up-to-date documentation for each client.
- Communicating effectively with clients, their families, and other healthcare professionals to ensure coordinated and comprehensive care, providing updates on clients' progress, and addressing concerns or questions.
- Transporting clients to appointments, community activities, and other necessary outings, ensuring clients' safety and comfort during transportation.
- Maintaining a clean and safe environment for clients, assisting with light housekeeping duties, including laundry, cleaning, and organizing clients' living spaces.
Requirements:
- A high school diploma or equivalent.
- Strong communication and interpersonal skills, with the ability to build positive relationships with clients and their families.
- The ability to work independently and as part of a team, demonstrating reliability and accountability.
- A valid driver's license and reliable transportation.
- Must pass a background check and drug screening.
- Must have a smartphone to access work-related applications and communicate with the team.
