Job Title: RN Hospice Team Leader

We are seeking a highly skilled and experienced RN to join our team as a Hospice Team Leader. As a key member of our leadership team, you will be responsible for the day-to-day operations of our home-based hospice patient care and management of staff.

Responsibilities:
  • Lead a team of RNs and other healthcare professionals in providing high-quality patient care
  • Develop and implement care plans that meet the unique needs of each patient
  • Collaborate with other departments to ensure seamless care coordination
  • Supervise and mentor staff to ensure they have the necessary skills and knowledge to provide excellent care
  • Monitor and evaluate the quality of care provided and make recommendations for improvement
Requirements:
  • 5+ years of experience as an RN in hospice or home health care
  • 2+ years of experience in a leadership or supervisory role
  • Advanced clinical assessment skills and knowledge of hospice care
  • Excellent communication and interpersonal skills
  • Ability to work independently and as part of a team
