Position Overview

Salary: Per Diem opportunity for a Licensed Master Social Worker (LMSW) in the Richmond area.

Company: Constellation Health Services is seeking a dedicated and empathetic Social Worker (LMSW) to provide support to our hospice patients. This role encompasses delivering care in both residential settings and Skilled Nursing Facilities.

Qualifications Required:

  • Possession of an LMSW license in Virginia with a minimum of one year of clinical experience; experience in Hospice, Acute Care, or Bereavement Counseling is preferred.
  • Access to reliable transportation and willingness to travel within a designated area to ensure optimal in-home patient care.

Key Responsibilities:

  • Deliver Clinical Social Work services to our hospice clientele, conducting social evaluations that assess family dynamics, caregiver capabilities, communication patterns, and potential neglect or abuse, while planning interventions based on these evaluations.
  • Provide education to patients and caregivers regarding disease processes, medication management, care plans, and tailored treatment strategies.
  • Ensure all documentation adheres to agency standards, utilizing the electronic medical record system, and submit all clinical documentation, time records, and additional information within the required timeframes.
