Sam's Club offers a unique opportunity for independent optometrists to establish their own practice within our stores. This partnership allows professionals to take control of their business, setting their own schedules and fee structures.

We provide the space, and you bring the expertise. As an independent lease holder, you will be responsible for patient care, including eye exams, vision correction, and general health services.

This is a chance to build your own business, with support from Sam's Club. You will have access to fair market value equipment and instrumentation, as well as administrative and opt-tech support services (if state laws permit).

A Doctor of Optometry (OD) or Ophthalmologist (MD/DO) with a valid state license is required. Additionally, General Liability Certificate and Professional Liability Insurance Certificate are necessary. A W-9 form is also required for tax purposes.

The estimated annual salary for this position is $200,000 - $400,000, depending on experience and location. Benefits include autonomy, flexibility, and the potential for long-term growth.
