Overview:

As a RN Case Manager at CommonSpirit Health Mountain Region, you will play a vital role in ensuring the best possible care experience for our patients. This includes advocating for patients, collaborating with interdisciplinary healthcare partners, and connecting patients to resources that help them maintain and build on positive outcomes.

Responsibilities:

  • Utilize clinical expertise, discretion, and independent judgment in assessing/reassessing, facilitating care coordination, utilization management, and patient advocacy.
  • Assure medical appropriateness criteria are met for status and level of care.
  • Collaborate with healthcare teams to develop and implement patient care plans.
  • Provide education and support to patients and their families to promote healthy outcomes.

Requirements:

  • Nursing Diploma or Associates Degree in Nursing required; BSN preferred.
  • RN License in Applicable State of Employment required.
  • 1 year of nursing or case management experience required.
  • Experience working with EMR preferred.
  • Working knowledge of regulatory requirements and accreditation standards strongly preferred.

Physical Location:

Ortho Colorado Hospital 3rd Floor

Pay Range:

$34.14/Hr. to $59.45/Hr.

Shift:

Weekends with the flexibility to work during week for sick calls
