Responsibilities
  • Provide occupational therapy services to patients in their homes, using evidence-based techniques and in coordination with other members of the healthcare team.
  • Evaluate patients' functional status, occupational therapy needs, and develop individualized plans of care.
  • Consult with physicians and other healthcare professionals to ensure comprehensive care.
  • Document patient progress and outcomes, and participate in staff conferences and committees as requested.
  • May supervise home health aides and occupational therapy assistants, as per LHC policy and state regulations.
Requirements
  • Current Occupational Therapy licensure in the state of Illinois.
  • Current CPR certification.
  • Current driver's license, vehicle insurance, and access to a dependable vehicle or public transportation.
