About the Role

At Pathways Home Health, Hospice and Private Duty, we are seeking a highly skilled Hospice Nurse Practitioner to join our team. This is a full-time position, working Monday through Friday, 8:30am-5pm, with a territory covering Santa Clara County.

The estimated annual salary for this role is $157,200 - $192,000, based on an hourly rate of $76.00 - $91.00/hour. Please note that this is a reasonable estimate and may vary depending on various factors.

Job Description

This Hospice Nurse Practitioner will work in coordination with the assigned Director of Hospice and Hospice Medical Director to evaluate the appropriateness of ongoing hospice care for identified patients. The successful candidate will coordinate care with other members of the hospice care team to ensure appropriate plans of care and recertification documentation. Additionally, they will serve as a consultant to the Palliative Care team for patients transitioning to hospice care.

Main Responsibilities:

  • Assess patients to determine their suitability for home health or hospice services.
  • Coordinate and communicate with Hospice Physicians to ensure ongoing appropriateness for Hospice program participation is documented and certified.
  • Provide consultative services to Case Managers regarding patients with chronic conditions and/or symptom management issues.
  • Assist patients and Case Managers with creating a smooth transition to Hospice services.
  • Document observations, clinical findings, and problems, as well as skilled interventions and responses, goals, and plans, according to policy.
  • Admit patients to service, assessing their physical, mental, and social status, and eligibility for hospice services according to hospice admission criteria.
  • Ensure all applicable services ordered are explained and offered, obtaining physician orders for services, equipment, medication as needed.
  • Assess the need for and coordinate services from and with all other IDG members, including Physician, Social Worker, Home Health Aide, Dietician, Volunteer, Spiritual Care, or therapy services (PT, OT, ST).
  • Participate in supervisory visits to nursing staff and communicate findings to the Nursing Manager.
  • Ensure Plan of Care changes are documented as indicated.
  • Assess the need for care beyond the current certification period, obtain required orders, and complete required documentation.
  • Complete required discharge procedures and documentation.
  • Participate in multiple Hospice interdisciplinary team conferences, including the Palliative Care Team if needed.
  • Maintain and update knowledge and skills required to provide quality hospice nursing care.
  • Assist in developing hospice and palliative care protocols, procedures, and policies as requested.
  • Participate in Quality Assurance/Performance Improvement activities and in-service education related to areas of practice as appropriate.
  • Follow established standards for point-of-service technology, documentation, and synchronization.
  • Use agency-provided telecommunication devices according to established guidelines.
  • Submit weekly visit schedules for assigned patients.
  • Submit all documentation according to agency requirements.
  • Meet established productivity standards.
  • Demonstrate established clinical competencies.
  • Assist in developing agency protocols, procedures, and policies as requested.
  • Evaluate, develop, organize, and deliver teaching materials for assigned home care patients as appropriate.
  • Participate in developing protocols or critical paths for patients on the Palliative Care Team and other chronic care programs. Evaluate areas where Hospice should be considered for inclusion in disease management protocols and outcomes.
  • Perform other duties as assigned consistent with skills and training, and the mission and goals of Pathways.
