Key Responsibilities- Develop and implement individualized care plans that prioritize quality of life, comfort, symptom management, and functional capacity.
- Provide direct nursing care to patients, including administering medications, performing procedures, and monitoring patient condition.
- Collaborate with interdisciplinary teams to coordinate patient care and address the physical, emotional, and spiritual needs of patients and their families.
- Communicate effectively with patients, families, and healthcare teams to ensure seamless care transitions and optimal patient outcomes.
- Bachelor's degree in Nursing or equivalent required.
- Current licensure as a Registered Nurse in the state of Kansas required.
- At least 3-5 years of experience in nursing, with a focus on palliative care or hospice preferred.
- Experience with serious illness conversations, goals of care conversations, and inpatient care preferred.
