Physical Therapist

As a skilled Physical Therapist, you will play a vital role in the physical therapy management of patients at Kaiser Permanente. You will work closely with the Director, Assistant Director, and Clinical Supervisor to ensure the highest level of care for our patients.
Key Responsibilities:- Evaluate patients and interpret examination findings to determine the nature and extent of dysfunction, with the goal of restoring their abilities to move and perform functional activities in their daily lives.
- Develop and implement a plan of care to achieve established goals, taking into account the patient's age, medical history, and dysfunction.
- Administer or prescribe modalities and other physical agents to promote healing and recovery.
- Design and implement exercise programs to help patients achieve their goals.
- Regularly reassess clinical signs and symptoms to determine the effectiveness of treatment and make adjustments as needed.
- Implement the treatment program through direct treatment of the patient, and may delegate a portion of the treatment plan to appropriate supportive personnel.
- Coordinate the treatment plan and participate in discharge planning with other healthcare personnel as appropriate.
- Provide education and advice to patients and their families to promote their understanding and involvement in the treatment process.
- Uphold Kaiser Permanente's policies and procedures, as well as applicable state and federal laws.
- Ensure accurate and timely documentation of physical therapy services provided, in accordance with state and federal requirements and relevant medical group policies and guidelines.
- Completion of an accredited PT training program with a Bachelor's, Master's, or Doctorate degree in Physical Therapy.
- Physical Therapist License - Applicant (California) required at hire OR Physical Therapist License (California) required at hire.
- National Provider Identifier required at hire.
- Basic Life Support required at hire from American Heart Association.
- Member of the American Physical Therapy Association (APTA).

- This is a per diem position with flexible hours.
