Key Responsibilities- Provide one-to-one care with patients in their homes
- Manage patient pain and symptoms
- Collaborate with healthcare teams to develop and implement patient care plans
- Communicate effectively with patients, families, and healthcare providers
- Document patient care and outcomes
- Graduate of an approved nursing education program
- Licensure as a Registered Nurse in the State of Illinois
- One (1) year experience as a professional acute care nurse
- Preferred: At least 1 year Hospice experience
- Medical, dental, and vision insurance - day one benefits
