- Conduct thorough assessments and evaluations to identify areas of improvement and develop effective interventions to enhance patients' mobility, endurance, and overall well-being.
- Collaborate with healthcare teams to ensure seamless care coordination and communication.
- Provide education and training to patients, families, and caregivers on proper techniques and strategies for managing chronic conditions and promoting healthy lifestyles.
- Document patient progress and outcomes, and communicate effectively with healthcare teams to ensure continuity of care.
- Current Physical Therapy licensure in the state of New Jersey.
- Current CPR certification.
- Must have a current driver's license and vehicle insurance, and access to a dependable vehicle.
