This role involves performing comprehensive assessments of client status, including physical, psychosocial, and environmental parameters. You will develop an individualized plan of care for each client, incorporating analysis of data and current scientific findings.

Key responsibilities include:

  • Performing comprehensive assessments of client status
  • Developing individualized plans of care
  • Providing interventions to achieve realistic client outcomes
  • Maintaining accurate records of client progress
  • Initiating referrals to other health team members as needed

We are looking for a qualified Registered Nurse (RN) with a graduate degree from an accredited school of professional nursing. You should have current licensure as an RN and current CPR certification. Reliable transportation is also required.
