About the RoleAs a Home Health RN Case Manager, you will provide high-quality, personalized care to patients in the comfort of their own homes. You will work closely with a team of healthcare professionals to develop and implement individualized care plans that meet the unique needs of each patient.
Responsibilities- Conduct thorough assessments of patients' physical and emotional needs
- Develop and implement comprehensive care plans that address patients' medical, social, and emotional needs
- Provide education and support to patients and their families on disease management, medication, and self-care
- Collaborate with healthcare teams to ensure seamless transitions of care
- Monitor patients' progress and make adjustments to care plans as needed
- A competitive salary range of $75,000 to $100,000 annually

- Active, unrestricted RN license in the State of Michigan
