At Maplewood Brewster LLC, we're seeking a dedicated Licensed Practical Nurse (LPN) to join our team as a Wellness Nurse LPN/RN. As a key member of our healthcare team, you'll play a vital role in delivering exceptional care to our residents, fostering a sense of community, and promoting overall well-being.

About the Role:

  • Supervise and support Resident Services Department associates, including C.N.A.'s (Resident Services Assistants), to ensure seamless care delivery.
  • Assist in supervising the medication program and support residents with self-administration of medication, ensuring their safety and comfort.
  • Maintain positive relationships with residents, families, and physicians, providing exceptional customer service and addressing concerns promptly.
  • Report significant incidents and changes in residents' needs to the Executive Director/Resident Services Director, ensuring timely interventions and resolutions.
  • Support the completion of resident assessments and service plans, collaborating with the Resident Services Director to ensure comprehensive care.
  • Assist with clinical assessments/recertifications per ALSA requirements, maintaining accurate records and ensuring compliance.
  • Support the Resident Services Director in hiring, managing performance, and training within the department, promoting a culture of excellence.
  • Participate in scheduled in-service programs, training sessions, and associate meetings, staying up-to-date on best practices and industry developments.
  • Assist with scheduling and staffing of Resident Services Assistants as needed, ensuring adequate coverage and resident care.
  • Provide coaching and mentoring to Resident Services Department associates, promoting growth and development within the team.
  • Support residents and families as needed, ensuring high levels of customer satisfaction and loyalty.
  • Assist with supervision of RSA staff during provision of care, maintaining a safe and supportive environment.
  • Perform other duties as assigned, contributing to the overall success of the Resident Services Department.

Compliance and Safety:

  • Follow emergency procedures, ensuring timely responses to critical situations.
  • Understand and adhere to safety practices and procedures, maintaining a safe working environment.

Communication:

  • Communicate effectively with supervisors and other staff members, ensuring seamless collaboration and information sharing.
  • Maintain positive working relationships with residents, their families, peers, and other staff members, promoting a culture of respect and empathy.
  • Encourage a positive work environment, fostering a sense of community and teamwork.

Education, Experience, Licensure, and Certification:

  • Current licensure as a Licensed Practical Nurse (LPN) in the appropriate state(s).
  • Basic first aid and CPR certification, ensuring readiness for emergency situations.
  • Two years of full-time or full-time equivalent clinical experience, with a preference for experience in assisted living or long-term care settings.
  • Experience and/or special interest in working with seniors, demonstrating a passion for geriatric care.

Physical Requirements:

  • In good physical and emotional health, free from communicable diseases.
  • Physically able to bend, reach, and lift up to 50 pounds, as needed.
  • Physically able to stand for extended periods, ensuring adequate stamina for resident care.
  • Ability to transfer residents and use a Hoyer Lift and other medical equipment, as required.

Miscellaneous:

  • Required to work weekends and holidays as assigned, ensuring adequate coverage and resident care.
  • May be required to work on shifts other than the one for which hired, demonstrating flexibility and adaptability.
  • May be required to work extended hours, ensuring timely interventions and resolutions.
  • Subject to exposure to infectious waste, diseases, and conditions, maintaining a safe working environment.
  • May be exposed to hostile and emotionally upset residents due to mental status, requiring empathy and understanding.
