Job Summary:

We are seeking a skilled Oncology Nurse Navigator to join our team at Christus Health. As a key member of our oncology department, you will play a vital role in coordinating comprehensive patient care and ensuring timely assistance and support for patients.

About the Role:

The Oncology Nurse Navigator is responsible for assessing the physical, psychological, and social needs of cancer patients, enhancing patient outcomes and satisfaction while navigating barriers to care. You will work closely with healthcare providers to deliver optimal, patient-centered care that addresses individual health concerns and promotes effective communication.

Responsibilities:

  • Cordination of care in a timely and efficient manner
  • Assessing physical, psychological, and social needs of patients
  • Enhancing patient outcomes, increasing satisfaction, and reducing cost of care
  • Providing expert communication over the telephone or in person
  • Performing patient assessment/reassessment
  • Transitioning patients to the right level of care based on their individual needs
  • Facilitating accessibility to care and promoting optimal patient-centered care

Requirements:

  • Registered Nurse (BSN preferred)
  • Current New Mexico Nursing License
  • BLS certifications issued by the American Heart Association
  • Considerable knowledge of healthcare facilities, policies, and issues
  • Ability to coordinate various functions and activities
  • Strong communication skills (written and oral)

Work Environment:

This role requires working in multiple settings, including inpatient, specialty, and primary care settings. The ability to handle stressful situations and possess excellent customer service skills is essential.

Estimated Salary Range:$65,000-$90,000 per year, depending on experience.
