Job Summary:

We are seeking an Occupational Therapist to join our home health team, where you will play a critical role in helping patients achieve their full potential. Your expertise will enable you to assess, plan, and implement individualized treatment plans that cater to each patient's unique needs and goals.

Responsibilities:

  • Develop and implement personalized treatment plans that address patients' physical, emotional, and cognitive needs.
  • Conduct thorough assessments and evaluations to identify areas of improvement and set realistic goals.
  • Prioritize effective communication with patients, families, and healthcare teams to ensure seamless care coordination.
  • Utilize evidence-based practices and cutting-edge technologies to deliver high-quality care.
  • Maintain accurate records and reports to track patient progress and inform future treatment decisions.
  • Stay up-to-date with industry developments and participate in ongoing education and training opportunities to enhance your skills and knowledge.
  • Collaborate with multidisciplinary teams to foster a culture of mutual respect, trust, and open communication.
  • Adhere to regulatory requirements and maintain compliance with relevant laws and standards.

Requirements:

  • Certified Occupational Therapist with a current and valid California license.
  • Minimum 2 years of experience in home health or a related field.
  • Strong understanding of state and federal regulations governing home health care.
  • Excellent communication, interpersonal, and problem-solving skills.
  • Ability to work independently and collaboratively as part of a dynamic team.
  • Familiarity with electronic medical records and practice management systems.
  • Fluency in Spanish a plus.
  • Capacity to adapt to changing priorities and deadlines while maintaining a commitment to exceptional patient care.
