At Optum, we're revolutionizing the way care is delivered. Our Senior Community Care program is a pioneering initiative that brings together a multidisciplinary team to provide comprehensive care to our highest-risk health plan members.

Key Responsibilities:
  • Conduct thorough assessments to identify members' needs and develop personalized care plans.
  • Manage members through acute medical situations, exacerbations of chronic conditions, and short-term sub-acute rehabilitation episodes.
  • Ensure accurate and complete documentation, including ICD-10 conditions.
  • Collaborate with the interdisciplinary care team to provide seamless care coordination.
  • Conduct advanced illness and advanced care planning conversations with patients and caregivers.
  • Provide education and counseling to patients and caregivers to empower them to manage their health.
Requirements:
  • Active and unrestricted license in the state of Texas.
  • Graduate of an accredited Master of Science Nursing or Doctor of Nursing Practice program.
  • 1+ years of clinical experience in practice.
  • Ability to obtain post-hire or current, active DEA licensure/prescriptive authority.
  • Ability to complete physical requirements of the job, including lifting a 30-pound bag and navigating stairs and dwelling conditions.
  • Access to reliable transportation for travel to member facilities within the Waco area.
  • Proficient computer skills, including documentation of medical information with written and electronic medical records.
Preferred Qualifications:
  • Understanding of Geriatrics and Chronic Illness.
  • Understanding of Advanced Illness and end-of-life discussions.
