Key Responsibilities:
  • Assess, communicate, and document residents' status accurately
  • Observe and report significant changes in resident behavior and health to the Resident Care Director, physician, family, and other relevant parties
  • Direct and supervise the daily work assignments of Resident Care Assistants
  • Closely mentor and guide Lead Resident Care Assistants in all aspects of their job
  • Coordinate care needs with community providers via an effective case management process
  • Effectively balance service demands with supporting resident independence
  • Recognize and assist in the prevention of elder abuse, neglect, and exploitation, and report to appropriate sources as needed
Requirements:
  • Current state license as an LPN and CPR certification
  • Graduate of an approved LPN program (per state requirements)
  • Minimum of 1 to 2 years of experience working as a nurse, preferably with the elderly
