As a Medicare Wellness Nurse, you will play a vital role in providing high-quality healthcare to our patients, focusing on completing Medicare Annual Wellness visits and supporting clinic nursing responsibilities.
Key Responsibilities- Medicare Annual Wellness Visits: Ensure understanding of CMS requirements and workflows, provide and facilitate competent healthcare, and ensure accurate documentation.
- Healthcare Provision: Provide health and wellness education to individuals and groups, act as a preceptor, and support the development of other staff and healthcare students.
- Leadership and Quality Improvement: Provide leadership for initiatives to improve quality of care and clinical outcomes, assist in the development of departmental standing orders, and ensure compliance with DNV standards.
- Telephone Visits and Coordination: Perform telephone visits with eligible patients, complete AWVs as assigned, and assist with coordination of care for patients aged 18-150.
- Administrative Tasks: Perform telephone triage, assist with in-office procedures, administer immunizations and medications, and ensure accurate documentation.
To be successful in this role, you will need to have a strong understanding of CMS Medicare Annual Wellness Visit requirements and workflows, as well as excellent communication and interpersonal skills. You will also need to be able to work effectively in a fast-paced environment and prioritize tasks to meet deadlines.
