Job Title:

Hospice Case Manager RN

About the Role:

We are seeking a compassionate and skilled Registered Nurse to join our team as a Hospice Case Manager. As a vital member of our interdisciplinary team, you will provide high-quality patient care and coordinate services to ensure the best possible outcome for our patients.

Salary: $80,000 - $110,000 per year

Responsibilities:
  • Administer skilled nursing care for clients of all ages in their place of residence.
  • Coordinate care with the interdisciplinary team, patient/family, and referring agency.
Requirements:
  • Graduate of an accredited Diploma, Associate, Baccalaureate, or Masters School of Nursing.
  • Two years of experience as an RN in a clinical care setting preferred.
  • Knowledge and experience with end-of-life disease processes and terminal symptom management.
