Key Responsibilities:
- Develop and implement effective care plans to ensure quality-based, cost-effective patient outcomes
- Collaborate with medical, nursing, and ancillary department staff to facilitate seamless care transitions
- Conduct thorough assessments and identify opportunities for improvement in patient care
- Communicate effectively with patients, families, and healthcare teams to ensure informed decision-making
- Participate in quality improvement initiatives to enhance patient care and safety
Requirements:
- Current licensure as a Registered Nurse in Rhode Island
- Minimum of 3-5 years related clinical and/or case management experience
- Certification in Case Management (CCM or ACM) within 24 months of employment
- Effective written and verbal communication skills
- Ability to interpret medical data and utilize level of care management
