Northwestern Medicine is seeking a dedicated Nurse Practitioner to join our family medicine department. This role focuses exclusively on outpatient care. The Nurse Practitioner will engage in a variety of responsibilities, including patient consultations, managing patient overflow, handling in-basket tasks, conducting sick visits, and executing patient outreach initiatives.
Work Schedule:
Monday to Friday (approximately 7:30 AM to 5:00 PM)
No nights, weekends, holidays, or on-call duties.
- Exhibit clinical proficiency and extensive knowledge through patient care activities, which may encompass diagnosing and managing a diverse range of health issues.
- Accountable for the health outcomes of a designated patient population.
- Collaborate with physicians and other healthcare professionals in areas of clinical expertise.
- Develop educational resources for patients, families, and staff within areas of specialization.
- Mentor graduate students in advanced practice and patient care methodologies.
- Actively engage in quality improvement and/or research relevant to the specialty area.
- Conduct thorough assessments of patients' health by obtaining health histories and performing physical examinations.
- Deliver direct clinical care to patients.
- Prescribe medications and therapies; order and interpret laboratory tests, x-rays, and procedures in accordance with clinical privileges.
- Evaluate patients to determine their need for additional healthcare services.
- Provide comprehensive education to patients and their families.
- Document all care and education provided in the medical record accurately.
- Refer patients to other disciplines as necessary.
- Serve as a consultant to other disciplines in areas of expertise.
- Integrate research-based knowledge with clinical expertise in decision-making processes.
- Maintain a professional demeanor in appearance and conduct.
- Advocate for patients, families, colleagues, and the community.
- Keep certifications and continuing education up to date as required for licensure.
- Participate in departmental committees or task forces as appropriate.
- Develop and implement educational strategies for patients and their families.
- Facilitate continuing education programs for staff.
- Provide formal education to both internal and external groups relevant to the specialty.
- Engage in quality improvement and/or research initiatives for specific patient populations as appropriate.
