Nurse Practitioner Opportunity

Matrix Medical Network offers a range of clinical services and expertise that provide primary care providers and at-risk health plan members with the tools and knowledge to better manage their health at home.
We meet our members where they are, breaking through traditional barriers to care, and helping older adults and other at-risk individuals enjoy a better quality of care, improved health outcomes, and identify chronic conditions that may otherwise go undiagnosed.
Key Responsibilities
- Conduct Adult and Geriatric Assessments, medical history, diagnosis, and treatment, health education, and documentation according to Matrix guidelines and protocols.
- Work collaboratively with physicians, case managers, social workers, family members, key caregivers, and ancillary medical personnel as appropriate.
- Collaborate with Primary Care Physicians on patient education and provide follow-up care.
- Provide services in a variety of settings, including home visits, skilled facility visits, and telehealth visits.

Qualifications
- Master's Degree in Nursing required, or commensurate experience and satisfactory completion of NP licensure.
- Current RN and NP licensure in the state of practice, with prescription authority or the ability to obtain it.
- Board certification by the AANP, ANCC, or AACCN in a Matrix-approved specialty.
- Current BLS, ACLS, or CPR certification.
- Minimum of 1 year experience as a Nurse Practitioner.
- Strong computer skills and familiarity with employee health/medical record software.
- Excellent verbal and written communication skills with patients, clients, and colleagues.
Travel Requirements
- Able to travel and possess a valid state driver's license and adequate automobile insurance coverage.
- Travel required to meet patients in their homes.
