Healthcare Manager

Open Door Family Medical Center is seeking a dedicated and compassionate Health Home Care Manager to join our team. As a key member of our care management team, you will work closely with adults with Medicaid who have chronic medical and behavioral health conditions to improve health outcomes, develop self-management skills, and reduce unnecessary emergency room/hospital visits.
The ideal candidate will have a strong background in healthcare and/or social services, with experience in case/care management. You will be responsible for conducting assessments, working with enrollees to establish care plan goals, providing health education, and coordinating medical and social services. This is a dynamic and rewarding role that requires strong communication and interpersonal skills, as well as the ability to work effectively in a fast-paced environment.
Responsibilities- Conduct thorough assessments to identify individualized needs related to psychosocial well-being, physical health, and the social determinants of health.
- Collaborate with enrollees to set goals and develop a care plan, monitoring progress and revising the plan as needed.
- Utilize Motivational Interviewing and other techniques to assist in goal setting and achievement.
- Coordinate and arrange for the provision of medical, behavioral health, social, and other relevant services, including appointment scheduling, reminders, and transportation booking.
- Accompany enrollees to appointments on- and off-site, providing support and guidance as needed.
- Collaborate with family members/caregivers, internal and external service providers, and other stakeholders to ensure comprehensive care.
- Model and coach enrollees on medical/behavioral service navigation, empowering them to become independent, effective healthcare consumers.
- Conduct home visits to provide services and assess the enrollee's living environment.
- Support enrollees in adhering to treatment recommendations from medical and behavioral health providers.
- Provide health education to assist with enrollees' understanding of physical and behavioral health conditions and treatment, linking them with evidence- and research-based resources.
- Discuss advance directives and healthcare proxies with enrollees and their families or caregivers.
- Maintain a caseload of 50 individuals, ensuring timely and effective care coordination.
- Complete all documentation as required, including data entry into the New York State data portal and Open Door's electronic medical record.
- Master's Degree preferred; candidates with a Bachelor's Degree and solid work experience will be considered.
- Fluent in English and Spanish is required.
- Minimum two years' experience in healthcare and/or social services settings; case/care management experience a plus.
- Valid NYS driving license and daily access to a car are required.
- Proficient computer use, including MS Office applications, especially Excel; experience with electronic health records or case management software a plus.
- Strong written and verbal communication skills, with a patient-centric and empathetic approach.
- Ability to navigate healthcare and community settings, as well as enrollees' natural settings.
- Collaborative and effective when working with a team of service providers.
- Responsive and comfortable in a fast-paced, high-accountability environment.
- Highly organized, self-directed, able to multi-task, and flexible.
- Effective problem solver, exhibiting excellent judgment and the ability to prioritize.
