About the Role

The Home Healthcare Medical Director will lead our team of healthcare professionals in providing high-quality, patient-centered care to seniors and individuals with complex needs.

Responsibilities
  • Lead a multidisciplinary team of healthcare professionals in delivering evidence-based medical care to patients in their homes, assisted living facilities, or independent living communities.
  • Develop and maintain comprehensive care plans that meet the unique needs of each patient.
  • Collaborate with caregivers, family members, and other healthcare professionals to ensure seamless communication and coordination of care.
  • Order and interpret laboratory and diagnostic testing as needed to inform patient care decisions.
  • Provide referrals to other healthcare practitioners when necessary.
  • Ensure accurate and up-to-date patient medical records.
Requirements
  • A licensed Nurse Practitioner with current National Board Certification and active state licensure.
  • Master's degree in Nursing (MSN) from an accredited institution.
  • At least 5 years of nursing experience, with a minimum of 2 years as a Nurse Practitioner.
  • Strong interpersonal communication skills, both verbal and written.
  • Excellent time management skills, with accountability for daily workflow.
  • Proficiency in medical instruments and equipment, including electronic medical record (EMR) systems.
  • Demonstrated knowledge of HIPAA and OSHA regulations.
  • Licensed driver with valid automobile insurance.
  • Ability to engage in moderate physical activity, such as handling objects up to 25 lbs., standing or walking for more than four hours per day.
