Job Title: Physical Therapist In House SNF PRN

Evolve Therapy Services is a leading provider of therapy management services, seeking a compassionate and skilled Physical Therapist to join our team at a Skilled Nursing Facility in St. Petersburg, FL.

Job Summary:

We are looking for a highly motivated and experienced Physical Therapist to provide high-quality physical therapy services to our residents. As a Physical Therapist, you will be responsible for evaluating residents, developing treatment plans, and implementing interventions to improve their functional abilities and quality of life.

Responsibilities:
  • Provide physical therapy services to residents in accordance with established standards of practice and company procedures.
  • Conduct thorough evaluations of residents to determine their physical therapy needs and develop individualized treatment plans.
  • Implement and document physical therapy interventions, including exercises, modalities, and other treatments as needed.
  • Communicate effectively with residents, families, and healthcare team members to ensure seamless care coordination.
  • Maintain accurate and timely documentation of resident progress and treatment plans.
  • Participate in interdisciplinary team meetings to discuss resident care and develop comprehensive treatment plans.
  • Stay up-to-date with the latest physical therapy techniques and best practices through ongoing education and professional development.
Requirements:
  • Master's or Doctoral degree in Physical Therapy from an accredited program.
  • Active Physical Therapy license in the state of Florida.
  • Minimum of 1 year of experience as a Physical Therapist in a Skilled Nursing Facility setting.
  • Excellent communication and interpersonal skills.
  • Ability to work effectively in a fast-paced environment and prioritize multiple tasks.
