Job Summary:
We are looking for a dedicated and compassionate Registered Nurse to provide high-quality care to our patients in the comfort of their own homes. As a Home Health Admission Registered Nurse, you will be responsible for conducting home visits, assessing patient needs, and developing personalized care plans to ensure the best possible outcomes.
Responsibilities:
- Conduct home visits to assess patient needs and develop personalized care plans
- Administer medication, wound care, treatments, and procedures as prescribed by physicians
- Follow up with and execute doctors' orders, accurately documenting observations, interventions, and evaluations
- Utilize electronic medical records (EMR) to document patient information and communicate with healthcare teams
- Travel to cases as assigned, ensuring timely and efficient delivery of care
Requirements:
- Graduation from an accredited and approved nursing program
- Minimum of one year of recent, verifiable clinical (medical/surgical) experience
- Prior home care experience strongly preferred, but not required
- Solid computer skills and prior experience with EMR systems
- Ability to travel to cases as assigned
