Job Title: Travel Nurse RN Case Manager

ProKatchers is seeking a skilled Travel Nurse RN Case Manager to join our team in Glendale, California.

Job Summary:

We are looking for a highly motivated and experienced Travel Nurse RN Case Manager to work in our Emergency Department. The ideal candidate will have a strong background in case management and be able to work effectively in a fast-paced environment.

Responsibilities:
  • Proactively identify potential patient admissions and patients with needs for discharge.
  • Work with Bed Center staff to identify inappropriate patient status issues (inpatient versus observation).
  • Resolve issues of incorrect status through discussions with ED and Admitting Physicians by obtaining additional information to support admission order or determine alternative level of care.
  • Coordinate discharges home from the ED and facilitate transfers to external placement facilities.
  • Maintain expert knowledge of Interqual Level of Care criteria.
  • Screen all ED patients to determine potential admissions and the need for a clinical or psychosocial intervention.
  • Discuss all potential admissions with ED physician appropriately.
  • Coordinate all discharges with a need for clinical intervention.
  • Develop and maintain care plans for active management of patients who frequently seek ED care (e.g. list of Primary Care Physicians (PCP) accepting new patients community resources for families managing frail/elderly) or who may be discharged/transferred from the ED.
  • Start discharge planning on admitted patients.
Requirements:
  • 3 years relevant experience.
  • Graduate of accredited nursing program, Bachelor of Science in Nursing required.
  • BLS Required.
