Position Overview:
Key Responsibilities:
- Conduct thorough health evaluations for patients across all age groups.
- Identify and manage a variety of medical issues, including long-term conditions like diabetes, hypertension, and cardiovascular diseases.
- Request and analyze diagnostic examinations.
- Authorize medications and therapeutic interventions.
- Create and execute personalized care strategies.
- Inform patients regarding their health issues and prescribed treatments.
- Collaborate with other healthcare professionals, including specialists and social service providers.
- Engage in initiatives aimed at enhancing patient care quality.
- Ensure precise and comprehensive patient documentation using EPIC EMR.
- Master's degree in nursing with specialization in advanced practice nursing (APRN).
- Certification as an APRN by the American Nurses Credentialing Center (ANCC) in internal medicine.
- Valid nursing license in the state of NH.
- Experience in delivering primary care to adults of various ages.
- Strong communication and interpersonal abilities.
- Capability to function autonomously as well as collaboratively within a team.
