Job Summary: We are seeking a skilled Physical Therapist to provide therapy services in a home setting.

Responsibilities:

* Evaluate and treat patients in a home setting
* Communicate with families, physicians, and other healthcare professionals
* Maintain daily, thorough documentation of services in the EMR
* Have your own equipment, including smart devices and blood pressure monitors

Qualifications:

* Graduate of an accredited college or university with a Doctoral/Masters/Bachelors degree in Physical Therapy
* Actively licensed as a Physical Therapist

Work Schedule: PRN (as needed)
