- Provide patient education and support to patients and their families, ensuring they understand their treatment plans and medication instructions.
- Collaborate with other healthcare professionals to ensure seamless care transitions and effective communication.
- Participate in quality improvement initiatives to enhance patient care and outcomes.
- Minimum one year of recent experience as a Home Health RN in a licensed agency.
- Registered Nurse License (California) and Basic Life Support certification.
- Clean driving record and proof of automobile insurance.
