Work Schedule:
Monday - Friday; 8am-5pm with flexible hours

On-Call Rotation:
Weekend on-call rotation required

Incentives:
  • Sign-On Bonus: $7,000
  • Student Loan Reimbursement: $3,000 for three years after 90 days
  • Mileage Reimbursement: $15 daily for nurses commuting over 25 miles

Role Overview:
The RN Care Manager is tasked with overseeing the coordination of patient care and services. This role emphasizes quality care through a collaborative approach that coordinates, monitors, and evaluates services based on patient needs. The RN will work closely with patients, families, care teams, payers, and external organizations to ensure a safe transition from the facility to the next care provider.

Qualifications:

  • Nursing diploma (associate's or bachelor's degree in nursing)
  • Preferred: One (1) year of clinical experience in a post-acute care setting
  • Preferred: Experience in case management, utilization review, and discharge planning
  • Preferred: Certified Case Manager (CCM) or Board Certification in Nursing Case Management (RN BC)

Expertise Required:

  • In-depth knowledge of practices, procedures, laws, regulations, and guidelines governing functions in a post-acute care facility
  • Ability to implement and interpret departmental programs, goals, objectives, policies, and procedures
  • Proficiency in all competency areas, including patient rights, safety, and sanitation
  • Maintain confidentiality of all proprietary and confidential information
  • Adhere to company policies, including harassment and compliance procedures
  • Exhibit integrity and professionalism by following Life Care's Code of Conduct and completing mandatory compliance training

Technical Skills:
Demonstrate efficient use of relevant computer systems, including data entry and retrieval.

Communication:
Serve as a liaison to external case managers, families, physicians, and community resources. Educate patients, families, associates, and other care providers.

Standards of Practice:
Implement care management standards, ethical performance, and functions relevant to care coordination. Proficient in reading, writing, speaking, and understanding the English language.
