Job Summary

We are seeking a skilled Home Health Physical Therapist to join our fast-growing team. As a key member of our clinical team, you will play a vital role in delivering high-quality physical therapy services to patients in their homes.

You will be responsible for planning and administering prescribed skilled physical therapy treatment and training for patients suffering from various injuries, illnesses, and functional disabilities. Your goal will be to help patients attain their highest level of physical function.

Responsibilities
  • Plan and administer prescribed skilled physical therapy treatment and training for patients
  • Evaluate patients' physical strengths to assist physicians in evaluating their level of function and recording findings
  • Develop and implement a conditioning/rehabilitation program consistent with physicians' plans of treatment and overall goals of the patient/rehab team
  • Provide written instructions/care plans that reflect current plans of care to Physical Therapy Assistants and Home Health Aide staff
  • Supervise/evaluate staff performance and monitor completion of documentation
  • Accurately document patients' care observations, interventions, and evaluations
  • Report patients' progress to physicians, Clinical Managers, staff, patients, and families
Requirements
  • Advanced degree of Doctor of Physical Therapy (DPT) from an institution accredited by the American Physical Therapy Association (APTA)
  • Current and unrestricted license to practice Physical Therapy in the State of California
  • Current CPR/BLS certification
  • Compliant with state healthcare regulatory requirements
  • Possess up-to-date knowledge of treatment practices and physical therapy programs
  • Ability to care for patients with different personalities
  • Ability to assess patient condition and work with treatment plans
  • Minimum of 1 year of recent Physical Therapy experience in an institutional setting
  • Minimum of 1 year of recent Physical Therapy experience with a home health agency
  • Licensed driver with vehicle in good working order, currently registered and insured in accordance with state and Neogen Care requirements
  • Strong skills in observation, verbal and written communication, problem-solving, basic math, Electronic Medical Record Software (Kinnser a plus), and basic computer skills

Estimated Salary: $120,000 - $160,000 per year, depending on experience and location. Location: Tracy, CA and surrounding areas.
