Primary Care Clinician for Home Risk Assessments

Matrix Medical Network is seeking a skilled Primary Care Clinician to join our team in Athens, NY.
This PRN/Part Time opportunity offers flexible scheduling and the chance to make a direct impact on the health and well-being of others.
Salary and BenefitsThe estimated salary range for this position is $2,300-$11,500 per month, based on an average 20-100 completed visits per month.
Additionally, you will have access to a mileage reimbursement plan and limited Voluntary Benefits.
Job DescriptionAs a Primary Care Clinician, you will conduct Adult/Geriatric Assessments, medical history, diagnosis and treatment, health education, physician referrals, case management referrals, follow-up and clear documentation according to Matrix guidelines and protocols.
You will work collaboratively with physicians, case managers, social workers, family members, key caregivers and any appropriate ancillary medical personnel as necessary.
Responsibilities include providing services in a variety of venues to include: Home Visits, Skilled Facility Visits, and Telehealth Visits.
RequirementsTo be successful in this role, you must possess:
- A Master's Degree or commensurate experience and satisfactory completion of NP licensure.
- Current RN and NP/PA licensure in the state of practice to include prescription authority or the ability to obtain prescriptive authority.
- Board certification by the NCCPA, AANP, ANCC or the AACCN in a Matrix approved specialty.
- 1 year experience as a Nurse Practitioner/Physician Assistant.
- Strong computer skills and familiarity with EMR software.
- Excellent verbal and written communication skills with patients, clients and colleagues.
Travel Requirements include having a valid state driver license and adequate automobile insurance coverage.
