We are seeking a highly skilled and dedicated Registered Nurse to join our team at West Bloomfield SNF, LLC as a Wound Care Specialist. The successful candidate will be responsible for providing exceptional wound care services to our residents, ensuring their comfort, dignity, and well-being.
Key Responsibilities- Wound Assessment and Management
- Conduct thorough and accurate wound assessments to identify and manage specific skin conditions, such as pressure injuries, diabetic, venous, or arterial ulcers, and traumatic or complicated wounds.
- Develop and implement individualized wound care plans, in collaboration with the interdisciplinary care team, to promote healing and prevent further complications.
- Collaboration and Communication
- Work closely with the care team, including physicians, therapists, and other healthcare professionals, to ensure seamless coordination of care and effective communication.
- Provide education and support to residents, families, and caregivers on wound care techniques, diagnostic results, and preventive measures.
- Documentation and Reporting
- Accurately document all wound assessments, treatments, and progress in the resident's medical record.
- Prepare and submit regular reports to the Director of Nursing and other stakeholders on wound care outcomes and trends.
- Professional Development
- Participate in ongoing education and training to stay current with best practices in wound care and management.
- Attend and contribute to wound care conferences, workshops, and other professional development opportunities.
- Education and Licensure
- Current state license as a Registered Nurse.
- Bachelor's degree in Nursing or related field.
- Experience and Skills
- Minimum one year of experience in wound care or a related field.
- Strong knowledge of wound care principles, practices, and regulations.
- Excellent communication, interpersonal, and problem-solving skills.
- Ability to work effectively in a fast-paced, dynamic environment.
