Job Title: Nurse (LPN or RN)Job Summary:

We are seeking a skilled and compassionate Nurse (LPN or RN) to join our team at Heartland Human Care Services. As a Nurse (LPN or RN), you will play a vital role in providing high-quality patient care and supporting our medical team.

Key Responsibilities:
  • Administer medications and immunizations under the direction of a nurse or physician.
  • Educate participants on immunizations, medications, and preventative care.
  • Observe participants post-treatment, chart, and report changes in conditions.
  • Take and record patient history and complaints, temperature, pulse, respiration, blood pressure, height, and weight.
  • Assist physicians and nurses with procedures in accordance with established protocols.
Requirements:
  • Graduate of a Licensed Practical Nursing program (LPN) or Bachelor of Science in Nursing (BSN) program.
  • One year of experience in nursing, case work, human services, or related field.
  • Active Illinois LPN or RN license in good standing.

Work Environment:

This role requires working on-site and may involve travel to multiple sites during the day. In-person meetings and trainings may be required on a regular basis.
