Salary:

$50,000 - $65,000 per year, depending on experience

Job Description:

We are seeking an experienced Registered Nurse (RN) to fill the role of Clinical Care Coordinator - Rehabilitation Services. In this position, you will be responsible for coordinating the completion of Minimum Data Sets (MDS) by all disciplines, reviewing data for accuracy, and formulating Resident Care Plans (RCPs) along with the interdisciplinary care plan team.

  • Maintain and follow a schedule of due dates for all MDS
  • Coordinate the completion of the MDS by all disciplines
  • Review the MDS data for accuracy and meet with appropriate staff as needed to assure accuracy
  • Complete the MDS and input into the computer
  • Verify that assessments have been transmitted and approved by the State in a timely manner
  • Correct any rejected records and prepare them for re-submission
  • Complete CAAs according to state guidelines
  • Formulate RCPs along with the interdisciplinary care plan team for all residents in accordance with their needs and within the required time frame
  • Schedule and conduct RCP meetings on a regular and timely basis
  • Communicate the RCP to appropriate staff members and notify supervisor if noncompliance by staff is noted
  • Work with all in-house and ancillary departments to assure understanding and compliance with the RCP
Required Skills and Qualifications:
  • Registered Nurse (RN) degree from an accredited School of Nursing
  • Current, valid RN license from the North Carolina Board of Nursing
  • Ability to read, know, and follow personnel, department, and facility policies and procedures
  • Experience with MDS-RAP and Care Planning functions
  • Prefer experience with RUG-IV and MDS 3.0
