1. Assess physician medication orders for suitability, dosage accuracy, potential drug interactions, and administration routes. Provide recommendations for therapy modifications to physicians and healthcare staff as necessary.
2. Maintain a comprehensive patient/resident database, ensuring it includes relevant medical history, clinical and laboratory data, and current medications, including over-the-counter and home remedies.
3. Ensure that necessary laboratory tests for monitoring high-risk conditions are ordered, assessed, and communicated with the healthcare team.
4. Collaborate with healthcare professionals to deliver clinical and technical support as required.
5. Document all interactions with physicians, nurses, and other healthcare team members, as well as patients and their families, in the medical records.
6. Implement policies and procedures for the safe procurement, storage, distribution, and disposal of pharmaceuticals and biological products.
7. Obtain and maintain all required documentation in compliance with federal and state regulations and company policies.
8. Report any medication-related errors to the Pharmacy Manager.
9. Assist facility staff with safe and effective practices regarding the procurement, maintenance, administration, and disposal of medications.
10. Perform additional tasks as assigned.
Qualifications
- Bachelor's Degree in Pharmacy; Pharm D preferred
- Minimum of one year of experience in long-term care settings
Licenses/Certifications:
As mandated by governmental regulations and business requirements
Skills:
- Detail-oriented and goal-driven
- Team player with strong oral communication skills
