NP Housecalls Clinician

Job Summary:

We are seeking a skilled Nurse Practitioner to join our Optum Housecalls team. As a Housecalls Clinician, you will conduct in-home assessments, evaluate patient health, and provide comprehensive care to patients in the comfort of their own homes.

Responsibilities:

• Conduct in-home assessments, including medical history review, vital signs, and comprehensive physical exams
• Perform evidence-based practice screenings, including point-of-care testing
• Identify diagnoses and develop care plans to address patient needs
• Communicate findings with patients' primary care physicians to ensure seamless care
• Provide education to patients on disease management, medication, and compliance

Requirements:

• Active NP license and national certification in Family, Geriatrics, or Adult-Gerontology Primary Care
• Ability to complete physical requirements of the job, including lifting and navigating various dwelling conditions
• Reliable transportation for home visits

Preferred Qualifications:

• Experience in gerontology, cardiology, internal medicine, or endocrinology
• Home health care or home visit experience
• Fluency in Spanish, Cantonese, Korean, Vietnamese, Polish, or other languages
