Job Summary

We are seeking a highly skilled Speech Language Pathologist to join our team at Life Care Center of Farmington. As a Speech Language Pathologist, you will be responsible for providing direct rehab care in speech therapy to patients upon physician referral.

Responsibilities
  • Establish, assess, and modify realistic, measurable, timely, and functional goals for patients
  • Develop and implement individualized speech therapy treatment plans
  • Conduct assessments and evaluations to identify patient needs and develop treatment plans
  • Provide direct speech therapy services to patients, including individual and group sessions
  • Collaborate with interdisciplinary team members to ensure comprehensive care
  • Document patient progress and maintain accurate records
  • Participate in quality improvement initiatives and maintain compliance with regulatory requirements
Requirements
  • Master's degree in Speech-Language Pathology from an accredited program
  • Current licensure in Speech-Language Pathology in the state of New Mexico
  • ASHA certification (CCC) or eligibility for Clinical Fellowship Year (CFY)
  • One year of experience in post-acute care or related setting preferred
  • CPR certification
