• Provide physical therapy services, including assessment, evaluation, and treatment plans.
• Communicate with other healthcare professionals to ensure seamless care coordination.
• Supervise and instruct non-professional home health aide staff in providing personal care to patients.
• Update care plans as needed and submit visit notes as requested.
• Perform OASIS time point assessments and discharge planning process per Medicare criteria.
• Attend staff meetings, team conferences, and educational in-services as required.

• A current, unencumbered State Physical Therapy License.
• Current driver's license, good driving record, proof of insurance, and reliable mode of transportation.
• One year experience as a home care professional Physical Therapist, with competency in performing comprehensive assessments and OASIS-C.
• The ability to make sound professional clinical judgment and assess patient needs.
• Proficiency in clinical skills and Microsoft Office.
