Job Summary

We are seeking a skilled Nurse Practitioner to join our team as a Home Risk Assessment Specialist. In this role, you will conduct comprehensive health assessments in the comfort of patients' homes, providing personalized care and support to help them manage their health.

Key Responsibilities
  • Conduct thorough health assessments, including medical history, diagnosis, and treatment planning
  • Develop and implement personalized care plans to address patients' unique needs and goals
  • Collaborate with healthcare providers, case managers, and social workers to ensure seamless care coordination
  • Provide education and support to patients and their families to promote healthy behaviors and self-management
  • Document all assessments and care plans in accordance with Matrix Medical Network's guidelines and protocols
Requirements
  • Master's degree in Nursing or related field
  • Current RN and NP licensure in the state of practice, including prescription authority
  • Board certification in a Matrix-approved specialty
  • Minimum of 1 year experience as a Nurse Practitioner
  • Strong computer skills and familiarity with electronic health records
  • Excellent communication and interpersonal skills
